BACK TO SCHOOL 2018-2019 

Checklist for parents / guardians 

** Everything should be turned in by Friday, August 17th ** 


Paperwork / forms will be available August 14th at Panther Pathway to Success Night. 

Paperwork will be sent home with students on August 15th/16th if it is not completed on August 14th. 


Student and Family Information (SaFIPacket: 

  • Demographics and Instructions – Complete / Update ALL THREE sections and SIGN ALL THREE 

  • Data Sharing – Update and SIGN  

  • Student Electronic Accounts – does not need to be signed or returned 

  • Health – Complete entire page and SIGN 

  • Family Census – Update all sections as necessary 


Required before student will be permitted to take home student computer on August 17th: 

  • Parent/Guardian Technology Use Responsibilities SIGNED 

  • Student Acceptable Use and Internet Safety Guidelines SIGNED 

  • Voluntary Device Protection Program ($20 if choose to participate) 



  • Panther Pack / Collective for Youth paperwork 

  • Yearbook Purchase $20  

  • PTO Membership $10  

  • Federal Lunch Application

Report to your PRIDE class to pick up class schedule.

·      Obtain student class schedule and meet teachers

·      Complete Student and Family Information (SaFI) packets

·      Turn in school/athletic physical forms

·      Complete REQUIRED paperwork permitting students to take computers home on Friday, August 17thSEE BELOW for more detailed information

·      Add lunch money to student accounts / check lunch status in cafeteria

·      Visit with school nurse regarding medications / physicals / special needs

·      Inquire about athletic seasons

·      Meet Panther Pack Staff and sign necessary paperwork for participation

School Supplies provided and/or needed:

·       All students are provided a Morton Binder with tab dividers and school agenda.

·       Families are asked to supply:  loose leaf paper, pencils, pens, highlighters, and three single subject spiral notebooks.

2018 – 2019 Student Computers

Exciting news!  Student computers will be distributed to students on Friday, August 17th… and students will be permitted to take the computers home!  This a HUGE responsibility.  Students will be responsible for bringing their fully charged HP Stream safely to school every day.  Students are provided binders that can be used to transport their computers in a safe way throughout the school year.

Students will only be allowed to take computers home with these two forms completed:

o   Parent/Guardian Technology Use Responsibilities

o   Student Acceptable Use and Internet Safety Guidelines

These forms will be available at Panther Pathway to Success Night (August 14th) and in the main office on August 15th, 16th and 17th.  Students without completed forms will not be allowed to take their computers home on Friday, August 17th.

The parent/guardian is responsible for the loss, theft or damage to the device.  The district has initiated a Voluntary Device Protection Program.  The parent/guardian may enroll in the Voluntary Devise Protection Program for the 2018-2019 school year by paying a one-time, annual fee of $20 per device.  Any parent/guardian declining to participate in the Voluntary Device Protection Program shall be responsible for the actual cost of the replacement or repair of any lost, stolen or damaged device.  Participation in the Program is Voluntary.

The Voluntary Device Protection Program covers the device only, not power cords, covers, etc.  The Program does not cover damages caused by:  unexplained loss or mysterious disappearance, failure to notify police if device is stolen, intentional acts, dishonest acts, theft from an unattended vehicle, unauthorized repairs or non-timely reporting.  More detailed information is provided on the forms required to be completed by August 17th.
